Sub: Issue of photo ID card to persons with disabilities (Divyangjan)
Ref: (i) Commercial Circular No. 18 of 2015 dated 19.03.2015
(ii) Commercial Circular No. 28 of 2016 dated 29.06.2016.
In partial modification of the instructions issued vide Board’s letter dated 19.03.2015 and 29.06.2016 referred to above, the detailed procedure may be amended as under:-
i. Following is added as Para 4(a) in Commercial Circular No. 18 of 2015:-
ARM offices wherever present may also be authorized to receive the application for issue of photo ID to persons with disabilities and they will forward it to the concerned DRM office for verification. After verification, the card issued under the signature of Sr. DCM may be sent back to ARM office in such cases and the applicants .can obtain their card from the concerned ARM office. The applicant will be informed telephonically to collect the card (contact number may be mentioned on the form by the applicant). The PwD or his/her representative (with proper authority letter) on his/her behalf can collect the card showing the original certificates.
ii. Para 10 of Commercial Circular No.18 of 2015 is substituted by the following:-
a) Loss of ID card: In case of loss of ID card, the applicant needs to submit an application for re-issue of card along with copy of FIR, authorized proof of identity and two passport size photographs to the office from which the card has been issued either in person, through his/her representative (with proper authority letter) or through post. The new card shall, however, be issued by the concerned authority on verification from their records.
b) Damaged/Mutilated/Torn ID card: The person with disability or his/her representative (with proper authority letter) on his/her behalf can approach the DRM or ARM office or send through post the application for re-issue of card indicating the reason for reissue and details of physically damaged/tom card like ID card no., Name, Card valid up to, Date of issue etc., if available, along with the authorized proof of identity and two passport size photographs. Since the original card is issued after due verification, there is no need of fresh verification of the disability from the hospital. The new card shall, however, be issued by the concerned authority on verification from their records and on surrendering of the damaged/torn ID card at the time of issue of new card.
The concerned authority must disable the surrendered card before issuing a new one. After disabling of a card, no fresh bookings would be possible but cancellation of reservations done prior to disabling of the card would be permitted by the system. A penalty of Rs.150/- may be charged for re-issue of card. The new card will have the same validity as that of surrendered card.
In case of shifting of residence of the individual beneficiary, the application for re-issue of the fresh ID card in the instances mentioned above can be submitted to the Divisional office of the current place of residence which will obtain records from the Divisional office which had issued the lost/physically damaged/torn/expired ID card earlier.
The applicant will be informed telephonically to collect the new card (contact no. may be mentioned on the application by the applicant). The person with disability or his/her representative (with proper authority letter) on his/her behalf can collect the card showing the authorized proof of identity.
iii. Para 8 of Commercial Circular No. 18 of 2015 and para (ii) of Commercial Circular No. 28 of 2016 is substituted by the following:-
Validity of the photo ID card is co-terminus with that of concession certificate.
2. List of acceptable documents as proof of identity, address and date of birth for the purpose of issue/re-issue of photo ID card is enclosed as Annexure.
3. Zonal Railways are requested to ensure effective implementation of the scheme under intimation to this office.

a) Photo Identity proof:-
i. Aadhaar card/downloaded Aadhaar (e-Aadhaar)
ii. Passport
iii. PAN card issued by Income Tax Department
iv. Voter ID card issued by Election Commission of India
v. Driving License issued by RTO
vi. Photo identity card having serial number issued by Central/State Government.
vii. Photo identity cards having serial number issued by Public Sector Undertakings of State/Central Government, District Administrations, Municipal bodies and Panchayat Administrations.
viii. Student Identity card with photograph issued by recognized School/College for their students.
ix. Photo passbook of running bank account ( Scheduled Public Sector Banks, Scheduled Private Sector Indian Banks and Regional Rural Banks only)
x. Credit cards issued by banks with laminated photograph.
xi. Ration/PDS photo card.
xii. Certificate of Identity having photo issued by Gazetted Officer or Tehsildar on letterhead.
b) Date of Birth Proof:-
i. Birth Certificate issued by the Registrar of Births and Deaths or the Municipal Corporation or any other prescribed authority, whosoever has been empowered under the Registration of Birth and Deaths Act, 1969 to register the birth of a child born in India,
ii. Transfer/School leavlng/Matriculation Certificate issued by the school last attended/ recognized educational board.
iii. Passportiv. Aadhaar card/downloaded Aadhaar (e-Aadhaar)v. PAN card issued by the Income Tax Department.
vi. Voter ID cardvii. Certificate of Date of Birth issued by Group A Gazetted officer or Tehsildar on letterhead,
viii. A declaration given by the Head of the Orphanage/Child Care Home on their official letterhead of the organization confirming the DOB of thel applicant.
c) Address Proof:-
i. Water Bill (not older than 3 months).
ii. Telephone Bill (Landline or post paid mobile bill- not older than 3 months)
iii. Electricity Bill (not older than 3 months)
iv. Property Tax receipt ( not older than one year)
v. Gas connection bill (not older than 3 months)
vi. Aadhaar card/downloaded Aadhaar (e-Aadhaar)
vii. Passport/ Passport of Spouse/ Passport of parents ( in case of minor)(provided the applicant’s present address matches the address mentioned in the spouse/parents)
viii. PAN card issued by Income Tax Department
ix. Voter ID card issued by Election Commission of India
x. Driving License issued by RTO.
xi. Photo passbook of running bank account ( Scheduled Public Sector Banks, Scheduled Private Sector Indian Banks and Regional Rural Banks only)
xii. Credit card statement ( not older than 3 months)
xiii. Post Office passbook/ account statement
xiv. Ration/PDS photo card
xv. Photo identity card having serial number issued by Central/State Government.
xvi. Photo identity cards having serial number issued by Public Sector Undertakings of State/Central Government, District Administrations, Municipal bodies and Panchayat Administrations.
xvii. Signed letter having photo from bank on letterhead/ signed letter issued by employer of registered company on letterhead/ signed letter issued by Recognized Educational institution.
xviii. Income Tax Assessment Order
xix. Registered Rent Agreement.
xx. Certificate of Address issued by Gazetted officer/Tehsildar on letterhead.
xxi. Certificate of Address issued by Village Panchayat head or its equivalent authority.
